The raeisf class file can be used to produce the RAE individual survey form.

To extract the code, do:

latex raeisf.ins

This will create the files raeisf.cls (class file) and raeisf.bst (accompanying
BibTeX style file.)

To extract the documentation, do:

latex raeisf.dtx
latex raeisf.dtx

Alternatively, if you use pdflatex instead of latex, the documentation will
have hyperlinks.
